The new findings are "pretty consistent with what we've seen before in this area in terms of demonstrating that people who experiment with electronic cigarettes, even if they swear at baseline that they would never smoke regular cigarettes, are at much more risk of transitioning to regular cigarettes", said Dr. Brian Primack, director of the Center for Research on Media, Technology and Health at the University of Pittsburgh in Pennsylvania. E-cigarettes, however, "provided greater satisfaction and were rated as more helpful to refrain from smoking than nicotine-replacement products", the study's authors wrote. Second-hand smoke can be especially harmful to children, and CDC estimates that second-hand smoke caused more than 7,300 deaths from lung cancer every year from 2005 to 2009.

Another concern is vaping's popularity with youths, as America grapples with a 78 percent surge in e-cigarette use among high school students from 2017-2018, which the US Surgeon General has described as an "epidemic". The journal was apparently so embarrassed about the positive results that it commissioned not one but two accompanying editorials slamming e-cigarettes, one offering questionable advice to doctors (insist smokers try and fail with NRT first; use low nicotine vaping products), and the other advocating for a vaping flavor ban.

There are an estimated 480,000 lives lost nationwide each year, and the American Lung Association states that close to 95 percent of smokers tried their first cigarette before the age of 21.

They addressed the possible long-term safety issues, saying that people's continued use of e-cigarettes "can be seen as problematic if e-cigarette use for a year signals ongoing long-term use, which may pose as-yet-unknown health risks".

Doctors from the United States - where experts have tended to be more cautious about e-cigarettes than those in Britain - pointed out that even with e-cigarettes 82 per cent of people failed to give up smoking. In fact, the study showed that when diacetyl is present in e-cigarettes, it is at a substantially lower level than what is found in a conventional cigarette. That compares with just 4 percent of kids who had not used any type of non-cigarette tobacco products.

The multi-centre trial, which involved nearly 900 smokers who also received additional behavioural support, found that 18.0 per cent of e-cigarette users were smoke-free after a year, compared to 9.9 per cent of participants who were using other nicotine replacement therapies. "Health professionals have been reluctant to recommend [e-cigarettes] because of the lack of clear evidence from randomized controlled trials", he told The New York Times.

For now, Borelli added, "the best way to quit smoking right now is to use FDA-approved evidence-based treatment as a first-line choice".
